To refer a patient to RD Dental, please complete and submit the below referral form in as much detail as possible and attach any x-rays.

We will be in contact with the patient shortly to discuss booking a consultation with them. We endeavour to keep you updated on your patient’s progress.

We will treat your patient with professionalism and care throughout their treatment with us, before discharging them back to your care.
